NEW ALGORITHM FOR DETERMINING THE REMODELLING OF THE HEART IN CHRONIC HEART FAILURE WITH PRESERVED EJECTION FRACTION

Objectives. To develop an algorithm for determining the remodelling of heart in chronic failure with preserved ejection fraction (HFpEF). Material and methods. In 2017-2019, on basis Healthcare Institution «1st City Clinical Hospital» Minsk, a one-stage (cross-sectional) comparative clinical instrumental study stratified random sample consisting 175 patients aged 71.0 (64.0; 78.0) years was performed. 2019-2021, State «Minsk Scientific-Practical Center Surgery, Transplantation Hematology» performed independent reproductive (examination) sample, which included 129 64.5 (58.0; 70.0) years. Criteria inclusion study: sinus rhythm; essential arterial hypertension; ischemic disease; HFpEF; presence voluntary informed consent patient to participate study. Exclusion criteria: primary mitral regurgitation, stenosis, valve repair or replacement, congenital defects, cardiomyopathies, acute respiratory diseases. Echocardiography 2D Speckle Tracking were using ultrasound devices Siemens Acuson S1000 (Germany), Vivid E9 (GE Healthcare, USA). Results. An HFpEF is based concept comprehensive, targeted echocardiographic assessment leading risk factors, pathofunctional mechanisms development, structural, functional abnormalities, intracardiac pulmonary hemodynamics, contains scientifically signs characterized by high diagnostic efficiency. The application echocardiography technology aimed at subthreshold systolic ventricular dysfunction, mechanical dispersion myocardial dyssynergy. Conclusions. developed reproducibility accuracy (93.8%), as has been shown examination makes it possible recommend its practical use determination HFpEF.
